The Only Metric That Matters in Digital Health

I recently had the chance to sit in on a conversation with a startup team building a care coordination app serving a very specific patient demographic.

What struck me wasn’t the technology, but their obsession with patient outcomes over feature delivery. It’s the way it should be. Yet it’s rarer than it should be.

The Trap Many Fall Into

In most environments, the pressure is similar. Ship fast, track engagement, and hit your sprint targets. Feature velocity becomes the proxy for progress, and the roadmap fills up with launches that move the metrics you can measure most easily.

When you’re building a consumer-facing app, optimizing for session time or registration rates is a reasonable north star. When you’re building a product that is supposed to help someone manage a chronic condition, coordinate their care, or make decisions about their health, those metrics are a starting point, not the destination.

The destination is the patient outcome.

It is surprisingly easy to lose sight of that. A team can spend an entire quarter shipping features, hitting delivery dates, and growing their active user count and still be failing the people they set out to serve.

If Your KPIs Don’t Measure Health, You Aren’t Measuring What Matters

The first place to look is your roadmap. If your success metrics are purely “delivery dates” and “active users,” you’re measuring output, not impact.

Digital health products require a different kind of instrumentation.

Build feedback loops directly into the product through Patient-Reported Outcome Measures that capture functional status, quality of life, pain levels, or mental health scores from the people using your product.

Partner with your clinical leads to track and review whether the product is actually moving the needle on what matters. Reducing hospital readmissions, improving medication adherence, supporting better care decisions, etc. These are hard metrics to collect and harder to attribute, but that difficulty is not an excuse to avoid them.

Finally, pay attention to the quality of engagement, not just the quantity. A user spending twenty minutes in an app isn’t necessarily a good sign. Are they there because it’s genuinely helping them, or because the experience is confusing? Time-on-feature can be a warning signal just as easily as it can be a success metric.

Build a Culture Oriented Around Outcomes

Measurement alone won’t shift behavior. The why has to be embedded in how your team works day to day.

Start by tying performance to clinical milestones rather than tickets closed. When the incentive structure rewards delivery, you get delivery. When it rewards outcomes, your team starts asking harder and more important questions before they ship.

Replace user stories with patient stories. Before sprint planning, take sixty seconds to share a real, de-identified account of how the product helped, or fell short for, an actual patient. It reorients the room quickly. Suddenly the conversation isn’t about what’s technically feasible. It’s about what actually matters.

Finally, give your team the authority to stop. Create an explicit, sanctioned ability for any team member to pause a release if they believe the pace is compromising patient safety or data privacy. Velocity is not more important than the people you are building for. Your team should feel empowered to act on that, not penalized for it.

Integrate Clinical Safety Into Your Definition of Done

Most development teams have a definition of done: security reviews, QA sign-offs, accessibility checks. In digital health products, clinical safety belongs on that same list, not as an afterthought, but as a hard gate.

Before fast-tracking any feature, audit the underlying data and algorithms for bias. There are documented cases of health technology performing meaningfully worse for specific demographic groups (pulse oximetry devices that are less accurate on darker skin being one of the more striking examples). Speed to market does not justify shipping a tool that underserves the patients who may need it most.

Require a clinical safety review for every major feature. Treat it with the same weight as a security review.

Build With Patients, Not Just For Them

One of the patterns I see repeatedly is product teams building what they believe patients need, based on assumptions that were never stress-tested against actual lived experience.

The startup I spoke with was deliberate about avoiding this. They embedded patient feedback directly into their process.

One approach worth adopting: an advisory panel of patients who review the roadmap quarterly. Not to validate decisions that have already been made, but to genuinely pressure-test what you’re building before you build it. Are you solving the daily friction points that actually affect their care? Or are you shipping features that feel innovative from the inside but don’t move anything that matters from the outside?

Move beyond A/B testing for clicks. Run longer-horizon pilots designed to answer the question that actually matters — does this feature change health behavior over time? A six-month longitudinal look at whether an intervention is working tells you something a two-week engagement spike cannot.

The team I spoke with reminded me that the most important thing a product team can do is hold the line on what the product is actually for.

It’s not registrations. It’s not session time. It’s not sprint velocity.

It’s whether the person on the other end of your product is doing better because of it.
